Chronic Disease Management

While a chronic disease may feel insurmountable, it can be easily managed with the right medical help. We have a growing multidisciplinary team onsite including a Credentialled Diabetes Educator, Sleep & Respiratory Physician and Dietitian to help support you in the management of your chronic disease.

Chronic Disease Management services include:

  • GP Management Plans

    A comprehensive GP management plan is crucial to the management of chronic disease. Your plan includes all the steps that should be taken over the next couple of years, including what you, your doctor and other health professionals are responsible for. With clear goal-setting, you’ll have a much clearer picture of how to manage your condition.

  • Enhanced primary care

    We can help establish an Enhanced Primary Care (EPC) plan to assist in the management of your condition. This plan is on the Medicare Benefits Schedule, and helps your GP to better plan your treatment over the coming months or years. An EPC is particularly beneficial if multiple health professionals are involved in your treatment.

  • Diabetes care

    Along with your GP, our fully credentialled diabetes educator is available to assist in the treatment of diabetes. We can help you and your family understand more about your condition, and provide the skills and knowledge to assist with diabetes self-management.

  • Asthma management

    Asthma affects around 2.7 million Australians (11% of the population), with cases ranging from mild to severe. We provide GP and specialist assistance to those suffering from asthma, including providing the skills and knowledge to manage your condition and improve your quality of life.

  • COPD

    Chronic Obstructive Pulmonary Disease (COPD) is a chronic condition affecting the lungs. We provide expert advice, medication and other forms of treatment to help better manage your COPD symptoms. With lifestyle changes and the right support, you can diminish the symptoms of COPD and breathe easier.

  • Home Medicine Reviews

    When you need multiple medications to properly manage your condition, it can be complicated to manage. A Home Medicine Review is designed to check all of the medications you’re taking at home. Not only is it a great opportunity to review the medicines you’re taking, but also to ensure you’re taking everything as directed. This service is also available for eligible carers looking after someone with a chronic condition.

  • Care coordination

    In line with your GP Management Plan or Enhanced Care Plan, we assist in coordinating the care you require from other health professionals. With referrals to trusted specialists and care services, we can help take the hassle out of managing your ongoing care.
